New Patient Registration

Anyone in England can register with a GP surgery. It’s free to register. You do not need to show ID, proof of address or immigration status to register as a patient.

If you’re homeless, you can give a temporary address, such as a friend’s address, a day centre or the GP surgery address.

We welcome new patients to register with us. Before registering, please check that you live within our practice boundary.

If possible we recommend that you register with us online. However, paper forms are still available if you need one.

Register as a Patient Online

Register online using the NHS GP Registration Service.

The registration process will take around 15 minutes to complete, you will need to do this in one session. You do not need your NHS number, but it could make registration easier.

The service is designed and run by the NHS, so your personal information is safe. It cuts our administrative workload and makes it easier for you to register.

Print or request a Paper Registration Form

If you would prefer to use a paper form, you can either request one from reception at one of our sites, or print one at home using the link below.

Once you have completed the registration form, please hand it in to reception at one of our sites.

Once we’ve processed your registration, you will receive a text message to let you know that you have been registered at the practice.

Find Your NHS Number

Sometimes when filling in online forms or speaking with our staff, you may be asked for your NHS number.

This online tool will help you find your number if you do not know it.

Please Note

You do not need to know your NHS number to use NHS services, but it can be useful to have it.

Other ways to get your NHS number

If you cannot get your NHS number online you can:

  • find it on any letter from the NHS like a prescription or appointment letter
  • call your GP surgery and ask them for your number

Temporary Patient Registrations

If you are not registered with a doctor but need to see one, you can receive emergency treatment for 14 days. After 14 days you will need to register as a temporary or permanent patient.

To register as a temporary resident, please visit one of our sites and request a GMS3 form to complete and hand back to reception.

You can register as a temporary resident with a GP in England if you are in the area for longer than 24 hours, but less than three months.
